Care Planning

When you contact us about our care services, we will arrange for our Care Assessor/ Co-ordinator to visit you in your home to talk about your needs and goals, so that we can put together your Care Plan. Of course, you are welcome to involve members of your family or a friend in this meeting.
Our Care Assessor/ Co-ordinator may also discuss the contract agreement as outlined by the local authority, if the authority has commissioned your care provision.
Your own Allan-Ross Care Plan will list all the tasks that you would like your carer to carry out and the days and times they will need to do this. The Care Plan will include emergency contact details, and instructions and information to allow you and your carers to work safely together (especially if you need to use specialist equipment such as a hoist).
Allan-Ross has a duty under the Health & Safety at Work Act (1974) to carry out a Risk Assessment, which will be kept in your Care Plan Book.
Your carers will keep a written daily record of your care in the Care Plan. You are welcome to read this at any time and change details if you feel the information is incorrect.

The Care Plan contains confidential information about you and needs to be kept in a safe place where the carer can find it. Carers are not allowed to discuss any details of their work for you with anyone else, unless it is with a Social Worker, District Nurse or GP.
Our carers will always maintain your right to privacy. However, there may be exceptional situations when carers are unable to keep information given to them confidential: they have a duty according to legislation and company policy to report any suspicions of abuse or discrimination to the Care Manager as soon as possible.
We recognise that people’s needs change and we will keep up to date with your needs by telephoning, visiting you and communicating with your carers. We will update your Risk Assessment and review your Care Plan whenever it becomes necessary (at least every 6 months).
